Cultivating and Maintaining Long-term Health

Primary Care and Internal Medicine

Primary Care

We believe primary care should be the first stop in your healthcare journey; it’s where you turn for guidance and support before you get sick, before you see a specialist, and before you need to seek hospital care. The comprehensive care provided by your primary care team ensures all of the treatments, medications, therapies and recommendations from various providers are as effective as possible. Our primary care team aims to build meaningful relationships with their patients to facilitate partnerships for overall health, wellness, and prevention through collaboration and appropriate integration of other care providers and specialty practitioners.

Internal Medicine

An internal medicine doctor, also known as an internist, is a primary care physician who specializes in treating adults 18 and older. Our board-certified internists are trained to handle everything from preventive medicine to the treatment of acute and chronic conditions. Your primary care team at CPPC includes our internist physicians, nurse practitioners and physician assistants that aim to establish long-term patient relationships centered on keeping you healthy and managing your healthcare needs.
